Become a credentialed provider with Wellpoint

We have a diverse network of exceptional healthcare professionals who ensure our members have uninterrupted access to the healthcare they need.

Credentialing is the process Wellpoint uses to evaluate and select licensed independent practitioners to provide care to our members. Wellpoint uses the Council for Affordable Healthcare (CAQH) ProView application for credentialing. This secure and private website reduces the amount of paperwork you need to fill out during the credentialing process.

CHOICES provider network only

Please complete the Joint MCO Universal Application to join our CHOICES provider network. Once completed, please email the application and requested credentialing documents to tnltssprovidercontracting@wellpoint.com to begin the process.

CHOICES and Employment and Community First (ECF) CHOICES provider networks

If you are interested in joining the ECF CHOICES provider network, or both the CHOICES and ECF CHOICES provider networks, applying and credentialing must be completed by the Tennessee Department of Disability and Aging (DDA). Instructions for joining are available on our Long-term services and supports (LTSS) page or by accessing the Tennessee DDA website and following the application instructions.
