Credentialing process
Providers interested in joining the Wellpoint provider network must have an active Iowa Medicaid ID and complete credentialing.
Obtain an Iowa Medicaid ID
Visit the Iowa Department of Human Services (DHS) website to enroll as an Iowa Medicaid provider.
Complete a credentialing application
Physicians/practitioners
Wellpoint encourages physicians and professional practitioners to use the CAQH ProView® uniform credentialing application . This free, secure platform lets you maintain just one credentialing profile for multiple organizations.
Facility/long-term services and supports (LTSS) providers
Facility and LTSS providers should complete and return the provider application and W-9 forms below.
